Marathon Health is seeking a full-time Certified Diabetes Education (CDE) to provide lifestyle and condition management coaching for the employees and families (ages 12+) at our employer-sponsored health and wellness centers in Charlotte, NC.

 

This position will be based among Marathon Health Network locations within a 25 mile radius.

 

You’ll treat individuals and families (ages  12+) in a healthcare setting that values quality of care, longer appointment times, and a more relaxed daily rhythm ― all so you can build and grow trusted relationships with your patients and help them reach their wellness goals.

Responsibilities

The CDE provides clinical expertise in diabetes management, in collaboration with the team of clinic providers.  Educate and create treatment plans for diabetic patients and successfully help them reach their disease management goals. Develop programming within the clinic location(s) to achieve positive change in the diabetic patient population.

  • Follow guidelines of the American Diabetes Association (ADA) and work with newly diagnosed diabetic patients on a comprehensive education plan encompassing nutrition, exercise and blood glucose monitoring.
  • Develop treatment plan and discuss proposed course of treatment with patient’s provider.
  • Collaborate regularly with providers, as well as other members of the team, including health coaches.
  • Work with poorly controlled diabetic patients or patients who have not reached their disease management goals, focusing on areas where improvement in control of disease could be accomplished.
  • Promote self-management to achieve individualized behavioral and treatment goals that optimize health outcomes.
  • Organize group programming for diabetes patients at various clinics and work sites, as deemed necessary.
  • Attend outreach activities such as health fairs for diabetes screening and health education.
  • Instruct patients and family members on how to administer insulin, self-monitor blood glucose, and medication education.
  • Assist in the development of policies and procedures for diabetes and health education guidelines.

Qualifications

  • Must be licensed as a certified diabetes educator with at least 2+ years of direct patient care experience treating a diabetic population
  • Registered Dietician and/or Health Coach Certification preferred
  • Knowledge and experience in health risk assessments, lifestyle risk reduction, disease management and health promotion required
  • Certified in BLS
